The clinics are open to all girls grades 9 and above.
The clinics are on Wednesdays from 6:00p-8:30p from January 2017 to May 2017.
This is the form that you need to completely fill out for each clinic you plan to attend. You will also need to have a Clinic Registration filled out on file, as well as a completed Health and Waiver form on file.
